Why is a Store Account Needed for Short Program?
You may be wondering why you need to create a store account if you're only wanting to get our free Short Program (the CFS/FMS symptom analysis program). There are several reasons:
1. | The Short Program software runs online at our website (it is not a downloaded application). You need a username and password to allow you to login and logout of our system — so that you can complete the Short Program in multiple sessions. |
2. | Although we ask for your billing and shipping address, we do not use it for anything, and we don't give your information out to anyone at all. We require this information only in the event that your physician — along with your authorization — determines that a formal medical record is needed for your case. This provides our software the ability to map your account onto our optional, more thorough, Full Program (but only if you specifically ask us to). |

Your Privacy is Safe with Us
Please note that no one will contact you and we will not give your contact information (email address, phone, etc.) to anyone. If you would rather not have your information kept in your account after you've finished the Short Program, you can simply send us an email asking us to delete your account information and we will do so.
A Credit Card is NOT Required
Since the Short Program is free, we do not need your credit card number. You may therefore leave this section blank when you create your account. If you decide you would like to purchase an item from our store sometime later, you can simply edit your account to add your credit card then.
